Above The Dirt Sports — Waiver & Release of Liability

Assumption of Risk

I acknowledge that softball is a physical activity involving inherent risks of injury, including but not limited to sprains, fractures, contusions, concussions, heat-related illness, and in rare cases serious or catastrophic injury. I voluntarily assume all such risks arising from my participation.

Release & Waiver

In consideration of being permitted to participate in Above The Dirt Sports activities, I, for myself and my heirs, executors, and assigns, hereby release, waive, and discharge Above The Dirt Sports, its officers, coaches, volunteers, sponsors, and affiliated venues from any and all liability, claims, or demands arising out of my participation, whether caused by negligence or otherwise.

Medical Authorization

I authorize designated Above The Dirt Sports representatives to secure emergency medical treatment on my behalf if I am unable to do so. I agree to be financially responsible for any medical treatment rendered.

Media Release

I grant Above The Dirt Sports permission to use my likeness in photographs and video taken during league activities for promotional purposes without compensation.
